JOB SUMMARY
The RN is charged with determining the appropriateness of potential clients and home supervisory visits of current clients.
JOB DUTIES
- Ensures compliance with company policy and all federal, state, and local regulating bodies and conditions of participation and Remains informed and updated on home health regulations and standards
- Ensures documentation is accurate and complete within each client's chart
- Develops, implements, reviews and revises policies and procedures to guide care delivery
- Coordinates all client services by reviewing plans of treatments, clinical and progress notes, and frequency of care.
- Communicates with physician's offices, discharge planners, hospitals, skilled nursing facilities and families regarding the client’s care and status
- Audits charts on a regular basis to assure proper maintenance of clinical records
- Perform other such tasks and assignments as directed by management/leadership of organization.
EDUCATION / SKILLS / ABILITIES / AVAILABILITY
- Bachelor of Science degree in nursing (BSN)
- Registered nurse licensure in the State of GA
- Two (2) years of work experience in Home Health Care
- Valid CPR, First Aid, AED
- Valid driver’s license, working vehicle and auto insurance.
- Ability to make recommendations to address and resolve issues.
- Ability to effectively deal with diverse groups and individuals and to maintain working relationships.
- Ability to organize workload and set priorities.
- Ability to operate office equipment, including, but not limited to computer, printers, copiers and fax machines. Knowledge of modern office practices.
- Knowledge and skill in records administration and maintenance.
WORKING ENVIRONMENT
- Works primarily out of the local office. Local travel within Chatham, Effingham and Bryan Counties 75%
